How to Apply

Get Started:

• Review the Mitral Foundation's Standards and Best Practices for Mitral Valve Repair Reference Centers on our Application Tools page
• Email our Program Director, [email protected], and request login information for your center to register for our Readiness Assessment Tool

Confirm Readiness:

• Using our Readiness Assessment Tool, provide initial information about your center's resources, mitral valve heart team, and minimum volumes as set forth in the Standards and Best Practices
• After confirming your center meets the criteria in the Standards and Best Practices, you will be invited to apply for recognition

Apply:

• Your center will submit detailed data for the past three years for all mitral valve repair procedures for primary (degenerative) mitral valve disease with prolapse (excluding severe mitral annular calcification (MAC) and leaflet calcification) performed at your center.  This data will be used to compute key performance measures which must be met to receive the Recognition Award

Be Recognized:

• If your center meets or exceeds the performance measures, your center will receive the Mitral Valve Repair Reference Center Award from the American Heart Association and the Mitral Foundation
